Reuses the ART-491 cutoff-vs-evaluated arithmetic, retargeted from a certification-period cut-off to a per-issue committed date. Emits a section 27.4 gate decision per issue and at the rollup: auto_pass / review_required / escalate / hold. Never itself judges that remediation was appropriate, timely, or sustainable; that judgment is a named human's section 27 approval record. Emits a hash-anchored OpenChainGraph v0.4 artifact.
