Agent Incident Record Composer
Composes a structured agent incident/failure record from caller-declared inputs: agent identity, an optional mandate hash, an incident description with an honest severity class, session evidence digests (OTel spans / in-toto links), remediation status, and an optional cross-link to an escalation record or a signed failure receipt. A missing agent identity degrades the record's claim strength rather than being silently accepted, and a malformed cross-link hash is flagged, not hidden.

Scope
This is an evidence FORMAT for an incident the caller declares: never an incident-detection system, never a determination of fault, never an insurance adjudication. Underwriters and insurers consume this record; we do not underwrite. Every output carries record_note restating that fence.
Severity class
Only affirming, warning, or contraindicated (the §22.9 AR4SI trustworthiness-vector tiers) are ever written to the artifact. Any other declared value is coerced to warning and flagged.
Cross-link, not lookup
escalation_cross_link can reference a §22.8.3 escalation record hash and/or a §22.9 signed failure receipt hash. This kernel checks the hash SHAPE deterministically; it does not fetch or cryptographically verify the referenced artifact.
